LICENSURE BY ENDORSEMENT
Opt 4.02 Application procedures. Ch. Opt 4 HistoryHistory: Chapter Opt 5 as it existed on March 31, 1989 was repealed, and a new chapter Opt 4 was created effective April 1, 1989.
Opt 4.01Opt 4.01 Qualifications. An optometrist holding a license, in good standing, to practice optometry in another state that has substantially similar requirements may become licensed and registered in Wisconsin if the applicant submits evidence satisfactory to the board that he or she satisfies all of the following criteria: Opt 4.01(1)(1) Has graduated from a program accredited by the Accreditation Council on Optometric Education. Opt 4.01(2)(2) Has passed the examination of the National Board of Examiners in Optometry as required under s. Opt 4.03 (2). Opt 4.01(3)(3) Has practiced optometry in the other state for at least 5 years. Opt 4.01(4)(4) Has passed the required state law examination administered by the board as required under s. Opt 4.03 (1). Opt 4.01(5)(5) Has never been disciplined by the licensing authority in any other state, territory of the United States, or another country for any misconduct or violations which evidence lack of competence to practice optometry in Wisconsin as determined by the board. Opt 4.01(6)(6) Does not have any pending complaints against the applicant or investigations of the applicant that relate to the practice of optometry. Opt 4.02(1)(1) An applicant for licensure under this chapter shall file with the board a completed application on a form provided by the board. The application shall include: Opt 4.02(1)(b)(b) Notice as to whether the applicant has been disciplined in any state in which he or she has held a license. Opt 4.02(1)(d)(d) A certified transcript of the coursework completed by the applicant submitted directly to the board from a program accredited by the Accreditation Council on Optometric Education. Opt 4.02(1)(e)(e) Verification of the applicant’s licensure submitted directly to the board by all states in which the applicant has ever held a license. Opt 4.02(2)(2) Applicants who have a pending criminal charge or have been convicted of any crime shall provide the board all related information necessary for the board to determine whether the circumstances of the pending criminal charge or conviction are substantially related to the circumstances of the licensed activity, subject to ss. 111.321, 111.322, and 111.335, Stats. Opt 4.02(3)(3) An application may not be considered by the board until the application is complete, including the requisite application materials under sub. (1), verification of examination scores from the National Board of Examiners in Optometry in accordance with s. Opt 4.03, or any other required information under this chapter. (3) Register January 2020 No. 769, eff. 2-1-20. Opt 4.03(1)(1) An applicant for a license by endorsement under this chapter shall take and pass the state law examination as set forth in s. Opt 3.02 (4). Opt 4.03(2)(2) An applicant for a license by endorsement under this chapter shall successfully complete one of the following: Opt 4.03(2)(a)(a) Parts I and II of the National Board of Examiners in Optometry, if the applicant has engaged in the practice of optometry for at least 5 years prior to January 1, 1996. Opt 4.03(2)(b)(b) Parts I, II, including passage of the Treatment and Management of Ocular Disease examination, and III of the National Board of Examiners in Optometry, if the applicant has engaged in the practice of optometry for less than 5 years prior to January 1, 1996, or if the applicant graduated from a program accredited by the Accreditation Council on Optometric Education after December 1, 1995.
