Opt 3.025 Reciprocal credentials for service members, former service members, and their spouses. Ch. Opt 3 HistoryHistory: Chapter Opt 3 and 4 as they existed on March 31, 1989 were repealed and a new chapter Opt 3 was created effective April 1, 1989.
Opt 3.02Opt 3.02 Application. Except as provided in s. Opt 3.025, an applicant for licensure as an optometrist shall submit all of the following: Opt 3.02(2)(2) Official documentation of graduation from a program accredited by the Accreditation Council on Optometric Education (ACOE). Opt 3.02(3)(3) Verification of passing parts I, II, including passage of the Treatment and Management of Ocular Disease examination, and III of the National Board of Examiners in Optometry examination. Opt 3.02(4)(4) Verification of passing the examination assessing knowledge of state laws and administrative rules regarding the practice of optometry. Opt 3.02(5)(5) If the applicant has an arrest or conviction record, documentation necessary for the board to determine whether the circumstances substantially relate to the practice of optometry, subject to ss. 111.321, 111.322, and 111.335, Stats. A reciprocal license to practice optometry shall be granted to a service member, former service member, or the spouse of a service member or former service member who meets the definitions under s. 440.09 (1), Stats., and who the board determines meets all of the requirements under s. 440.09 (2), Stats. The passing score on each examination is determined by the board to represent minimum competence. The board may adopt the recommended passing score of the examination provider.
