DC 1.01DC 1.01 Forms. All claimants, respondents or members of formal or informal patient compensation panels shall, in the presentation and the conduct of matters before the patient compensation panels, use the following forms which have been prepared by and may be obtained from the administrative director of courts:

DC 1.01(2)(2) Notice of Health Care Provider
DC 1.01(3)(3) Assignment of Controversy to Panel
DC 1.01(4)(4) Answer to Submission of Controversy
DC 1.01(5)(5) Cross-Complaint
DC 1.01(6)(6) Request for Hearing by Formal Panel
DC 1.01(7)(7) Stipulation for Hearing by Formal Panel
DC 1.01(8)(8) Petition, Consent & Order Appointing Guardian ad Litem for Settlement of Causes of Action Pursuant to s. 655.06, Stats.
