Section 1 ATCP 21.21 (1) (c) 2. is amended to read:
ATCP 21.21 (1) (c) 2. Any state or country, or any delineated area within a state or nation country, which the U. S. department of agriculture responsible state agency has declared to be infested with thousand cankers disease.
Section 2 ATCP 29.11 (3) (a), (b) and (c) are amended to read:
ATCP 29.11 (3) (a) If the applicant sold less than $25,000 of the product during the preceding year for use in this state, $3.50 $2.80.
(b) If the applicant sold at least $25,000 but less than $75,000 of the product during the preceding year for use in this state, $120 $96.00.
(c) If the applicant sold at least $75,000 of the product during the preceding year for use in this state, an amount equal to 0.75% 0.60% of gross revenues from product sales during the preceding year for use in this state.
Section 3 ATCP 29.15 (4) (b) is amended to read:
ATCP 29.15 (4) (b) An agricultural chemical cleanup surcharge of $28 $22.40.
Section 4 ATCP 29.20 (6) (b) is amended to read:
ATCP 29.20 (6) (b) Beginning with the license year that begins on January 1, 2008 2015, an agricultural chemical cleanup surcharge of $38 $30.40.
Section 5 ATCP 29.25 (5) (a) 1. and 2. are amended to read:
ATCP 29.25 (5) (a) 1. A license fee of $40, except that the license fee is $30 for each of the license years beginning January 1, 1999 and January 1, 2000.
2. An agricultural chemical cleanup surcharge of $14 $11.20.

The common chemical or brand name of each pesticide that may be applied.
A copy of the pesticide label for each pesticide that may be applied. The label will tell you the pesticide active ingredient, use directions and safety precautions.
Each date on which pesticides will be applied.
A contact name, address and telephone number where you can obtain further information about the pesticide application.
If you want this information in writing, we will provide it free of charge before we make the application. Whether or not you ask for this pre-application information, we will provide you with the following information when we complete the application:
The name, and license number and certification number of the individual who made or directly supervised the application.
