AB195-AA1,1,4 1. Page 2, line 8: delete the material beginning with “assigned" and ending with “physician" on line 9 and substitute “determined by a physician at birth and reflected on the birth certificate”.

AB195-AA1,1,7 2. Page 3, line 16: delete the material beginning with “assigned" and ending with “physician" on line 17 and substitute “determined by a physician at birth and reflected on the birth certificate”.
