DCF 57.37(7)(d) (d) Differences of elevation, including platforms, walkways, balconies and open sides of stairways shall be protected by railing at least 36 inches in height and with bars no greater than 4 inches apart.

DCF 57.37(8) (8) Water. If an infant under 6 months old is in care, the annual test of private well water required under s. DCF 57.40 (3) (a) 2., shall include testing for nitrate levels. If nitrate levels are determined to be over 10 milligrams per liter, the licensee shall give notice to the department field office that serves the group home within 48 hours.

DCF 57.37(9) (9) Nutrition and feeding.

DCF 57.37(9)(a) (a) Food shall be available to infants and toddlers in accordance with the meal patterns consistent with those specified in Appendices B and C, as applicable.

DCF 57.37(9)(b) (b) If a microwave is used to heat or prepare food, procedures for heating and cooking infant formula, milk, or food in a microwave oven shall be posted near the microwave.

DCF 57.37(9)(c) (c) Bottles shall not be propped. A child unable to hold a bottle shall be held whenever a bottle is given.

DCF 57.37(9)(d) (d) Infants and toddlers shall not be put to bed with a bottle.

DCF 57.37(10) (10) Medical care. Each child shall receive routine physical examinations and immunizations pursuant to s. 252.04, Stats., and ch. DHS 144, by a licensed medical professional.

DCF 57.38 DCF 57.38 Resident records.

DCF 57.38(1) (1) The licensee shall maintain on the premises a record for each current resident. Each record shall contain all of the following information:

DCF 57.38(1)(a) (a) Resident’s first name, last name, and alias, if any, date of birth, and gender.

DCF 57.38(1)(b) (b) Recent photograph of the resident.

DCF 57.38(1)(c) (c) Voluntary placement agreement or court order or both as appropriate.

DCF 57.38(1)(d) (d) Referral information such as court reports and assessments from the placing agency.

DCF 57.38(1)(e) (e) Name, address and telephone number of the placing agency, parent, guardian, or legal custodian that is responsible for the resident.

DCF 57.38(1)(f) (f) Name, address, and telephone number of the person or placing agency and physician to be called in an emergency.

DCF 57.38(1)(g) (g) Reasonable and prudent parenting decision records required under s. DCF 57.245 (2) (e).

DCF 57.38(1)(h) (h) Treatment plan and reviews and post-discharge plan.

DCF 57.38(1)(i) (i) Current medical information including all of the following:

DCF 57.38(1)(i)1. 1. Medical consent and signed releases.

DCF 57.38(1)(i)2. 2. Name of physician and dentist.

DCF 57.38(1)(i)3. 3. Dates of medical and dental examinations and recommendations for follow-up care.

DCF 57.38(1)(i)4. 4. Immunizations.

DCF 57.38(1)(i)5. 5. Illnesses and accidents and dates of each.

DCF 57.38(1)(i)6. 6. Medications and treatments received and dates of each.

DCF 57.38(1)(i)7. 7. Allergies, including allergies to food or medication.

DCF 57.38(1)(i)8. 8. Physical limitations.

DCF 57.38(1)(j) (j) Name of school and current grade.
