AB36-AA2,1,11 At the locations indicated, amend the bill as follows:
AB36-AA2,1,2 21. Page 4, line 1: after “2." insert “a.".
AB36-AA2,1,3 32. Page 4, line 4: after that line insert:
AB36-AA2,1,8 4“b. The self-assessment questionnaire shall state and the patient shall
5acknowledge on the self-assessment questionnaire that a hormonal contraceptive
6patch or a self-administered oral hormonal contraceptive is not protection against
7sexually transmitted diseases, which may have a significant negative impact on
8reproductive and general health.
AB36-AA2,1,129 c. The patient shall acknowledge on the self-assessment questionnaire that it
10is strongly recommended that the patient annually meet with a physician or other
11medical professional to discuss the patient's prescribed hormonal contraceptive
12treatment and other routine preventive care.”.
AB36-AA2,1,13 133. Page 4, line 25: after that line insert:

1“5. The rules promulgated under subd. 1. shall permit any qualified pharmacy
2employee to provide a self-assessment questionnaire required under subd. 3. a. and
3administer a blood pressure screening required under subd. 4. a. if a pharmacist
4reviews the results of the self-assessment questionnaire and the blood pressure
5screening before prescribing and dispensing any hormonal contraceptive patch or
6self-administered oral hormonal contraceptive to a patient.”.
AB36-AA2,2,8 74. Page 5, line 15: delete the material beginning with “the first day" and
8ending with “after publication," on line 16 and substitute “January 1, 2023,”.
