4“b. The self-assessment questionnaire shall state and the patient shall
5acknowledge on the self-assessment questionnaire that a hormonal contraceptive
6patch or a self-administered oral hormonal contraceptive is not protection against
7sexually transmitted diseases, which may have a significant negative impact on
8reproductive and general health.

c. The patient shall acknowledge on the self-assessment questionnaire that it
10is strongly recommended that the patient annually meet with a physician or other
11medical professional to discuss the patient's prescribed hormonal contraceptive
12treatment and other routine preventive care.”.

1“5. The rules promulgated under subd. 1. shall permit any qualified pharmacy
2employee to provide a self-assessment questionnaire required under subd. 3. a. and
3administer a blood pressure screening required under subd. 4. a. if a pharmacist
4reviews the results of the self-assessment questionnaire and the blood pressure
5screening before prescribing and dispensing any hormonal contraceptive patch or
6self-administered oral hormonal contraceptive to a patient.”.

(2) (a) In this subsection:

91. “Drug” has the meaning given in s. 450.01 (10).

113. “Pharmacist” has the meaning given in s. 450.01 (15).

12(b) The department of health services shall research and determine whether
13pharmacists could safely prescribe and dispense drugs or drug products in addition
14to those products prescribed and dispensed under s. 450.095.

15(c) The department of health services shall submit a report to the appropriate
16standing committees of the legislature upon request that includes a summary of the
17results of the research under par. (b) and all of the following:

181. A list of all drugs and drug products that the department believes may be
19prescribed and dispensed in a similar manner to hormonal contraceptives under s.
20450.095.

212. A description of any standards that the department of health services
22believes are necessary to allow pharmacists to safely prescribe and dispense the
23drugs and drug products listed in subd. 1.”.
